We are a team of experienced children’s nurses offering community visits, support and education for Children and Young People with medical needs.
We aim to improve the quality of life of Children and Young People with additional health needs by providing care in community settings, such as home and school, and try to prevent or reduce hospital admissions where possible.
We provide short term nursing care and treatment, as well as ongoing support to Children, Young People & their families to help them gain confidence in managing their condition.
We liaise with a range of professionals in the hospital and community settings to help give consistent care and advice.
We are experienced in managing a wide range of conditions including gastrostomy and nasogastric tube care, respiratory support including oxygen monitoring, wound care, intravenous (IV) therapy, blood sampling, continuing care assessments, shared oncology care and care at end of life.
We work with a range of other health professionals, including:
- Speech & Language therapists
- Occupational Therapists
- Physiotherapists
- Bowel & bladder nurse specialists
- Audiology
- Child development services
- Social Care
This means we can involve other agencies for extra support where needed, to ensure your family get the best care and experience.
A nurse will contact you to discuss your child’s needs and arrange a visit once we have received a referral from a healthcare professional.
We can provide visits in home and school settings as well as telephone consultations if appropriate and will always try to arrange a time that suits you and your family. We will decide together when, where, and how often we need to meet, depending on the needs of your child and family and the treatment needed, taking into account the advice of the referring clinician.
We will keep other professionals involved in your child’s care informed of their progress.
Student nurses have placements with our team so we may ask for your consent for a student to visit with us.
