For outpatient referrals, there is a supported discharge service to facilitate safe discharge from hospital to the patient’s home. These referrals can include:
- From Consultant-led Rheumatology service for functional assessment, sleep and fatigue management, and splinting
- From Consultant-led Orthopaedic service for hand splinting and therapy
- Cardiac rehabilitation (as part of a multi-disciplinary service)
The Community Therapy team is made up of Physiotherapists, Occupational Therapists, Therapy Technical Instructors and Administration Staff.
The team deliver rehabilitation to people in their own home for those recovering from illness, surgery, deterioration of a long-term condition or a loss if function. They also provide falls assessments and group falls exercise programmes within 3 locations.
