District Nursing

The District Nursing Service will provide nursing care to patients who are registered with a local GP, over 18 and who have been assessed as needing to be nursed at home.

The District Nursing service specialises in the complex management of patients in the community. All senior District Nurses hold the specialist qualification (District Nurse or Community Specialist Practitioner qualification CSP) in District Nursing.  Each team is led by a district nursing sister/charge nurse who is responsible for leadership, assurance of standards of care delivery, maintaining patient safety and establishment of effective relationship with general practices, secondary care providers, social care and other local care providers.

District Nurses play a significant part in caring for people, who have been assessed as having a clinical need and whose nursing care is more appropriately delivered at home. The range of clinical need and care is wide and varied and becoming more complex due to the acuity and dependency of patients.

As well as providing direct patient care, District Nurses  work with patients and other health and social care agencies to promote independence and support patients to manage their own condition. They also work with family members and advise them on how best to give care to their relative.
