We have a number of community beds offering, nursing and therapeutic interventions and reablement care to proactively bring people up to their potential for wellbeing and self-care, ad hoc specialist assessments and end of life care.
The service provides an element of ‘step up’ care to reduce avoidable admissions and ‘step down’ care to reduce extended length of stay.
Users are generally regarded as low dependency but need inpatient treatment and benefit from a multi-disciplinary approach to their recovery which cannot be provided in their home base.
The community wards are Advanced Clinical Practitioner led with medical oversight.
