Cardiac Rehabilitation is a six week programme of exercise and education designed to help patients manage their heart condition and improve their quality of life.
It is delivered by a specialist Cardiac Rehabilitation team which includes Physiotherapists, Nurses, Dietitians, Pharmacists and Occupational Therapists.
Cardiac Rehabilitation is beneficial for people who have recently experienced one or more of the following:
- Heart attack
- Coronary artery bypass graft
- Angioplasty
- Aortic valve replacement
- Angina
The six week course aims to equip patients with as much knowledge as possible about their condition and how to manage it. The course covers exercise, education & advice, emotional support, informal & practical advice on healthy eating, managing stress and medication. At the end of the six week programme, patients should have more confidence and improved ability to return to a normal fulfilling life.
The majority of referrals come from hospital on a patients discharge or from GPs and consultants. However, any healthcare professional can refer patients into the service or contact the team for advice.

The Heart Failure Team provides specialist care and advice to patients, their family members and their carers about Chronic Heart Failure. By working alongside our acute sector colleagues we provide care in community based nurse led clinics or if you are housebound within your own home. Our care is designed to help you best understand and therefore manage your condition which will hopefully lead to an improved quality of life.
